Investigation Summary
At 2:15 p.m. on August 14, 2012, Employee #1 was standing in front of the forklift where the pipe was located. It is unknown whether Employee #1 was trying to adjust or remove the choker around the pipe. Employee #1, who was also the foreman, was working with a crew of five employees that were installing a 24" high-density polyethylene (HDPE) pipe into an excavation that was approximately 200 ft. long and 5 ft. in depth. This job was for a fire water line project from the tank farm to the Total Refinery. The pipe that was being installed was of the same length of the excavation. At the time of the incident the pipe was on the ground along the trench and one end of the pipe was being inserted into the trench using a choker attached to the bucket of a track hoe (some portion of the pipe had already entered the trench). On the opposite end of the pipe was a Lull Forklift where the pipe was slightly suspended from the forks. While the pipe was being inserted into the trench, there was a small portion of sand placed into the trench simultaneously to cover the pipe. During the installation the foreman, was standing near the excavation, but opposite of the equipment. Employee #1 noticed a choker around the end of the pipe on the end closest to the forklift and it is suspected that the foreman walked around to the other side of the excavation to remove the choker from the pipe. Within the same action of him touching the choker, the pipe shifted and the force of the pipe hit Employee #1 and knocked him into the excavation with the pipe landing on top of him. The employee was crushed and killed by the pipe.
