Investigation Summary
At approximately 5:15 p.m. on July 31, 2012, Employee #1, a warehouse worker, was walking out of the rear exit of the building. She turned to talk to a coworker, who was operating an industrial truck (Nissan forklift, Model Number MLPL02A25LV). The coworker, who had not been trained to operate the forklift, stopped when Employee #1 passed under the elevated load. The load was unstable and unsecured. After the coworker stopped the forklift, the load of unsecured particleboards shifted and slid forward, falling off the forks. There were 11 sheets of particleboards, measuring 61 in. wide by 97 in. long by 0.75 in. thick with each weighing approximately 126 pounds, for a total of approximately 1386 pounds. The load fell and struck Employee #1 and caused unspecified injuries. Employee #1 was transported to a trauma center, where she was pronounced dead approximately 45 minutes later.
