Investigation Summary
At approximately 12:00 a.m. on April 26, 2012, Employee #1 was using a window finishing machine to smooth the corners of a finished window frame. As he operated the machine, he lost consciousness and collapsed from a heart attack. Emergency medical personnel were summoned. They arrived, but were unable to revive him and the died.
