Investigation Summary

At 2:55 p.m. on July 5, 2012, an employee, a semi-tractor driver had detached the 53-ft semitrailer at the USPS NDC dock door #2, and had exited the vehicle, when the Freightliner CL120 Sleeper Tractor Semi-Truck begin (Serial/Vin: 1FUJA6CG75DN58238) to roll away from the trailer. The employee chased after the rolling Freightliner CL120 Tractor Semi-Truck. When the driver caught up to the tractor and attempted to climb aboard while it was still moving. The truck had traveled approximately 230 feet, when the employee slip, tripped, and fell. The tractor rear dual wheels rolled over the employee's left arm & shoulder. The employee got up and moved to the curb, out of traffic and the tractor. The trailer continued to roll an additional 168 feet, where it hit a gate/guard house and fence. The employee was hospitalized and required surgery for his injuries. The employee later died approximately 30 hours following incident.
