Investigation Summary
At approximately 7:30 a.m., on June 20, 2012, an employee and coworkers arrived with their equipment at the site to pave a church parking lot. The preparation of the parking lot began with the employee operating the milling skid loader to remove the old asphalt. The preparation work took approximately two hours and several breaks for water were taken by all workers as needed. The first load of asphalt arrived at approximately 10:30 a.m. and it took 35-45 minutes to pave the first section of the lot. The workers took a lunch break and rested while waiting for the next load of asphalt to arrive approximately one hour later. The second load arrived at approximately 1:20 p.m. and took approximately one hour to pave the remainder of the lot. The employee and a coworker operated the Leeboy paver. At approximately 2:15 p.m., the paving was complete and they started to clean up the site. The employee walked to the end of the alley and sat down on a wall in the shade while others were cleaning up the site. After a short period of time, the workers saw the employee walking around when he collapsed. Two men driving by in a vehicle saw the employee collapse and stopped to render assistance and called the emergency medical services. The coworkers tried to help the employee until emergency medical service arrived. The emergency personnel arrived and after assessing the employee, transported him to the Washington Hospital Center. The employee suffered from what was believed to be heat related illness and was unconscious. He never regained consciousness. At 10:26 p.m. the employee died. The weather conditions on that day were extremely hot and humid. Weather data obtained from the NOAA website indicated the ambient air temperature at 2:35 p.m. was 98 F with the relative humidity at 35%. Using the heat index calculator from this web page the heat index at this time was 102F.
