Investigation Summary
At approximately 9:49 a.m. on May 1, 2012, Employee #1 was preparing a mandrel f or assembly into a large framework of an aircraft body frame that would be used as the mold for a carbon fiber structure. The mandrels were of different sizes a nd shapes necessitating the work platform to be modified depending on the mandre l being prepared. The platform was approximately 7 ft high. She had just started work and was applying tape to the mandrel, when she stepped into a floor openin g in a work platform. The floor opening was 89.75 in. wide and 84 in. deep. Part of that opening was covered by the mandrel extending over the floor opening. Em ployee #1 fell through the floor opening, falling approximately 7ft and receivin g multiple rib fractures, a fractured clavicle and a shattered sternum. She also stuck her head on the concrete floor and received concussion injuries including swelling of her brain. Employee #1 was transported to a medical center with fra ctures to the upper body and head injuries. At the hospital, she underwent surgi cal procedures and received additional medical treatment and was then hospitaliz ed.
