CRUSHED BY FALLING PAPER ROLL

ON JULY 8, 1985, AT APPROXIMATELY 11:30 AM, EMPLOYEE #1 WAS RETURNING A ROLL OF PAPER-TYPE MATERIAL TO A CONVEYOR AFTER HE HAD CUT THE ROLL TO THE RIGHT SIZE. THE ROLL WAS APPROXIMATELY 70 INCHES IN DIAMETER, 72 INCHES IN LENGTH AND WEIGHED APPROXIMATELY 7,400 LBS. THE EMPLOYEE FAILED TO FIRST INSTALL A STOPPING DEVICE ON THE SIDE OF THE TABLE ON WHICH THE ROLL WAS TO BE LOADED. AS A RESULT, WHEN THE ROLL WAS LIFTED ONTO THE TABLE IT BEGAN TO ROLL OFF THE OTHER SIDE OF THE CONVEYOR BELT. EMPLOYEE #1 AND EMPLOYEE #2 TRIED TO HOLD THE ROLL ON THE BELT BUT COULD NOT. EMPLOYEE #2 JUMPED OUT OF THE WAY, BUT EMPLOYEE #1 WAS STRUCK AROUND THE HEAD AND CRUSHED AGAINST ANOTHER ROLL. EMPLOYEE #2 WAS TREATED FOR SHOCK AT NEARBY MEDICAL FACILITY. EMPLOYEE #1 WAS KILLED.
