FINGER AMPUTATED IN SEALING MACHINE, GUARD DISABLED

EMPLOYEE #1, WHO WORKS AS A TABLE COORDINATOR, IS REQUIRED TO SEAL PLASTIC BAGS CONTAINING ONE OF THE PRODUCTS WHICH THE COMPANY MANUFACTURES. SHE DOES THIS BY HOLDING THE PLASTIC BAG AND INSERTING ITS TOP INTO A SEALING MACHINE. SOMEONE TAPED THE HINGED GUARD, WHICH COVERED THE POINT OF OPERATION OF THE SEALING MACHINE, INTO THE "UP" POSITION. EMPLOYEE #1 INADVERTENTLY PLACED HER LEFT INDEX FINGER INTO THE POINT OF OPERATION, WHERE IT WAS AMPUTATED.
