Employee crushed and killed when truck bed fell

Between approximately 8:30 and 9:00 a.m. on August 30, 1985, Employee #1 had finished unloading plywood from the bed of the company's 1979 GMC 9-ton flatbed truck. The bed was in the UP position and the motor was off. Employee #1 apparently got under the unsupported bed and was either inspecting the undercarriage or was tampering with the lift mechanism when the bed fell. He sustained crushing injuries to his upper back and lower neck areas. Employee #1 was killed.
