THE OPERATOR (EMPLOYEE #1) WAS USING A METAL BAR TO CHECK THE SLACK IN THE MANDRELL. THE BAR SLIPPED, CAUSING THE EMPLOYEE TO CUT HIS HAND WHEN IT WAS STRUCK AGAINST THE SLITTER KNIFE.
