EMPLOYEE #1 WAS LIFTING A LARGE PIECE OF WOOD WHEN HIS FINGERS SLIPPED OFF THE PIECE. IN ATTEMPTING TO REGAIN HIS GRIP HE CAUGHT HIS FINGERS BETWEEN THE PIECE OF WOOD AND A NEARBY METAL MACHINE.
