Investigation Summary
Employee #1 and Employee #2 were working from a work platform suspended from a crawler crane. The wire rope came loose from the block and hook holding the platform. The platform fell about 30 feet. The block and hook assembly, which weighed over 300 lb, fell on the employees crushing them. Employee #1 was killed and Employee #2 was hospitalized with broken bones.
