EMPLOYEE #1 WAS INJURED WHEN THE PERSONNEL CARRIAGE HE WAS RIDING IN FELL OFF THE FORKS OF A POWERED INDUSTRIAL TRUCK. HE FELL ONTO A STOCK RACK AND THEN ANOTHER 15 FEET TO THE FLOOR.
