EMPLOYEE KILLED WHEN HE SOMEHOW ENTERED A PAPER PULPER

EMPLOYEE #1 SOMEHOW GOT INTO THE PAPER PULPER (PROCESSING VAT). THE PULPER CONTAINED WATER AT 130 DEGREES FAHRENHEIT AND PIECES OF PAPER STOCK. THE PULPER WAS 14 FEET IN DIAMETER AND 10 FEET DEEP. THE PULPER WAS GUARDED BY A GUARDRAIL AND GATE. EMPLOYEE #1 WAS SO BADLY MANGLED WITH MULTIPLE INJURIES THAT HE COULD NOT BE IDENTIFIED UNTIL AN AUTOPSY WAS PERFORMED. NO ONE SAW HOW EMPLOYEE #1 ENTERED THE PULPER.
