Investigation Summary

On February 15, 2014, Employee #1 was going outside during break time. It was snowing and he slipped on the stairs and injured his ribs. He was treated on-site and was taken to the Emergency Room the next day, where he was admitted into the ICU with difficulty breathing. He was x-rayed where they discovered four fractured ribs on his right side. They also discovered that he had a bruised colon and had a lot tension from air & gas build-up. Surgery was performed but, after 60 seconds into the surgery, Employee #1 had no pulse. The State Medical Examiner performed a preliminary autopsy which determined that the death was due to complications with the fall. It was determined Employee #1 had a mild heart attack, inflammation of the colon and a pin point perforation of the colon.
