Investigation Summary
On February 28, 2014 Employee #1, with R & L Carriers Shared Services LLC., was found collapsed and unresponsive on the dock area shipping and receiving docks. Two emergency responders trained in first aid and CPR performed CPR on Employee #1 and summoned Emergency Medical Services. The emergency responders noted that Employee #1 did not have a pulse and was not breathing. Based on the medical examiner's report, Employee #1 died of natural causes. This case was closed as "no inspection," based on the medical examiner's report stating that the fatality was not work related.
