Investigation Summary
At approximately 9:20 a.m. on November 6, 2013, Employee #1 was trimming a palm tree on the grounds of a dolphin habitat that was located at a hotel. As he worked, he lost his balance and fell. He was not wearing fall protection. Emergency medical personnel were summoned. They arrived and transported him to the trauma unit of a medical center, where he succumbed to his injuries and died around 4:00 p.m.
