Investigation Summary
At approximately 7:15 p.m. on January 11, 2014, Employee #1, a production worker, was gathering trays from pallets. A coworker was operating a forklift nearby. As Employee #1 retrieved the trays, the forklift, moving in a reverse direction, struck and ran over her. Employee #1 sustained injuries to her left and right feet, left, left leg, and her lumbar spine. Emergency medical personnel were summoned. They arrived and transported her to a hospital, where she was treated and released the same day.
