Investigation Summary
At approximately 2:36 p.m. on December 23, 2013, Employee #1 was operating a Ridgid R4110 10-in. Compound Miter Saw to cut down medium-density fibreboard (MDF) baseboard. The baseboard was to be installed in an apartment of a building. While operating the miter saw with his right hand, he used his left hand to secure the baseboard against the miter's fence. His left hand (palmar or anterior side) made contact with the moving saw blade, which caused severe avulsions to his index and ring fingers and amputated his middle finger. Employee #1 was transported to a medical center and later transferred the same day to another medical center in another city, where he underwent surgery and was admitted for nine days.
