Investigation Summary
At approximately 7:30 a.m. on February 13, 2014, Employee #1 was performing some maintenance for a belt on a conveyor system, tightening a chain. Worker #1 turned the system on, believing that Employee #1 had cleared the area. When the conveyor belt started to move, it caused Employee #1 to fall back against the belt and the machine caught his left leg. Paramedics were called and he was treated on-site for injuries he received to his left shin and ankle before being transported to the UCSD Trauma Center.
