Employee Falls from Ladder Fracturing Wrists, Facial Bone

On December 2, 2013, Employee #1, a maintenance worker with Catholic Diocese of San Jose, was building a shed with a coworker. Employee #1 was holding a four ft by eight ft by 12 ins. piece of particle board on rafters of the shed, while standing six ft up on an extension ladder that was extended to about ten ft. The ladder was not tied off. The coworker had been holding the bottom of the ladder but let go for just a minute to go get the nails needed to attach the particle board to the roof rafters. At that time, a wind gust came up, pushing the particle board up into the air, causing Employee #1 and the ladder to fall backwards onto the pavement. Employee #1 received two broken wrists, broken bones around his nose, and bruises all over. Employee #1 was hospitalized.
