Investigation Summary
At 10:00 a.m. on January 21, 2014, Employee #1, a 22-year-old male with Blommer Chocolate of California LLC, manually loaded two pallets in the palletizer magazine of the automatic palletizer, Model Number EZY-150, Serial Number EZY06-54891. At approximately 10:03 a.m., one pallet jammed. So Employee #1 took off the top of the jammed pallet machine and placed it by the side of the machine. Employee #1 then reached inside the pallet conveyor and used his right arm to manually push the pallet. As Employee #1 pushed the pallet, the sensor detected that the pallet had cleared and the dispensing lift plate cycled, crushing Employee #1's right mid forearm between the frame and dispensing lift plate. Employee # 1 sustained an isolated crush injury to the right forearm with some paresthesia and no fracture. Employee #1 was admitted on January 21, 2014, treated and discharged on January 28, 2014 from Eden Medical Center. Based upon the interviews conducted and documentation developed during the course of this investigation, the employer did not ensure that Employee #1 de energized power to the palletizer prior to removing the jammed wooden pallet.
