Investigation Summary
On December 23, 2013, Employee #1, a food truck server, was at a commissary, looking for a pair of boots to wear. A manager asked him to wear a pair of boots. He tried finding one on the first floor of the commissary, but there was none, so he asked a coworker, who told him to check the second-story floor (mezzanine level) of the warehouse. Employee #1 traveled upstairs to the second floor and found some boots, but could not fit a size. He then decided to look further at the other side/wing of the second floor. As Employee #1 cross towards to the other side, he stepped on a canvass that he thought was covering a floor, but was not. Employee #1 fell through the unprotected side of the second-story floor, falling from the mezzanine level and fracturing his left wrist. Employee #1 was transported to a medical center, where he underwent surgery and was then hospitalized.
