Investigation Summary
At approximately 10:05 a.m. on March 5, 2014, Employee #1 was operating a forklift. As Employee #1 drove the forklift down on a ramp, its rear tire went over the edge of the ramp, causing the forklift to tip over. He sustained a serious crushing injury to his lower left leg. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized.
