Employee Loses His Balance and Falls from a Ladder

On June 4, 2013, Employee #1 of John Jay Coolidge was working as a grip at a production studio. Employee #1 was on a ladder adjusting lighting fixtures on a 14 feet grid. Employee #1 lost his balance and fell 12 feet to the ground resulting in a fatal injury. At the time of the accident, Employee #1 was working with four other crew members.
