Investigation Summary
On June 28, 2013, Employee #1, a server, was working at a restaurant. He was carrying dishes to the kitchen. As he walked, he slipped and fell on a wet floor. Not all wet and or greasy floor areas were covered with mats to prevent employees from slipping. When he fell, Employee #1 sustained a right wrist fracture and laceration, which was from a dish that had broke. Employee #1 was transported to a medical center, where he underwent surgery on the fracture and laceration and was then hospitalized.
