Investigation Summary
On August 5, 2013, Employee #1 was flaring PVC piping. This process required placement of two flanges held together by two clamps prior to the flaring process. He held two flanges together with his hands rather than applying the clamps. As a result, his right middle finger was crushed between two flanges.
