Employee Is Crushed Between Truck and Substructure

On August 7, 2013, an employee was working as a flagger for the move of a rig substructure by tandem truck. As the substructure was being unloaded at the new location, the employee got between the substructure and the back of the truck. As the front of the substructure came down off of the back of the truck, it moved forward several feet trapping the employee between it and the truck. The employee was killed.
