Employee Falls Backward, Dies

On June 1, 2013, Employee #1 was working using a wheel barrel. The employee went to lift the wheel barrel, which was partially full of materials. Employee #1 paused for a second and then fell backwards. Employee #1 was pronounced dead at the scene. The cause of death was not determined.
