Employee Is Struck by Falling Metal Plate and Is Injured

On July 7, 2013, Employee #1 was assisting an equipment operator with setting down a 1 in thick 8 ft by 20 ft metal plate over a trench. The metal plate slipped and struck Employee #1 in the left leg, causing a compound fracture and soft tissue damage. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized for his injuries.
