Investigation Summary
On August 8, 2013, an employee was assisting to clip a new wire rope onto the drum of the overhead crane's 50 ton auxiliary hoist, when the drum turned unexpectedly. As the employee stepped back, he was caught by the movement of an unguarded line shaft. The employee was killed.
