Employee Crushes Hand While Checking Axle Tolerances

At 07:35 a.m. on September 7, 2013, Employee #1 was checking tolerances on machined axles and wheels for trains on an assembly line. The wheel set consists of two train wheels and an axle that weighs approximately 3,320 lbs. He released a wheel set to roll on its own and then realized that it was rolling to fast. Employee #1 ran to the rolling wheel set, slipped and fell to the ground on one side near the rolling wheel set. The set ran over his left hand causing traumatic crush injuries.
