Investigation Summary

Investigation Nr: 202458493
Event: 08/12/2013
Employee Is Injured When Run over by Machinery

At approximately 1:47 p.m. on August 12, 2013, an employee was working alone in a field and using the equipment to perform a regularly assigned task of planting vegetable seeds. The employee was operating the tractor when he noticed the 4-row planter sled implement was not working properly. The seed was not being buried by the planter implement. He descended the tractor while it was moving slowly and climbed on to the implement. While climbing on the implement he slipped and fell in the implement and was dragged several feet under the drums on the rear of the implement. The employee sustained traumatic asphyxia, bilateral pulmonary contusion, right knee contusion and nasal bridge laceration. The employee was in the hospital for approximately two days. The employee had been operating a John Deere Tractor, Model Number 7130; Serial Number L07J30D613574, at the time of the incident. The 4-row planter sled implement, Model number SLP 4-36; Serial number 0081, was manufactured by B.W. The implement was approximately 9-ft long and 12-ft wide. There were four individual drums that were attached to the rear of the implement used to level the soil after the seed was planted.
