Event: 10/19/2012
Teacher Tears Achilles Tendon, Later Dies

At approximately 10:30 a.m. on October 19, 2012, Employee #1, a 39-year-old female teacher with Randolph County Board of Education Dba Archdale-Tr fell while playing kickball with her students. Employee #1 fell face / front body first while running at or close to second base and slid a short distance on the dirt. Employee #1 suffered a tear to her Achilles tendon (left leg) and was treated at Regional Physicians Walk-In Clinic - West in High Point, North Carolina. Employee #1 had surgery to repair her Achilles tendon on November 1, 2012 and her left leg and foot was placed in a cast. On November 30, 2012, Employee #1, was to return to have the cast removed however, that morning, Employee #1 collapsed on the toilet. Emergency Medical Services was summoned and several attempts failed to revive Employee #1. No autopsy was requested or conducted. Employee #1 was an organ donor, while the donor team was harvesting organs a massive intra-abdominal bleed was found. The Medical Examiner report established the fatality as job-related. The Medical Examiner's report arrived at the Raleigh NCDOL office on about April 16, 2013. The report was received at the Winston Salem, NC NCDOL field office on April 22, 2013 and assigned for investigation on April 24, 2013.
