Investigation Summary
On October 17, 2012, Employee #1 was using a Black and Decker 4.5 in. grinder, (Model Number G950 Type 1) modified by removing the grinding wheel and guard, replacing it with an approximately 6 in. rip saw. The employee was remodeling something in the basement. He also works in the kitchen as a cook. Employee #1 cut himself when the saw blade met the employee's left hand. Employee #1 reported to an area hospital, where three fingers on his left hand were amputated. Employee was hospitalized for three days and released.
