Investigation Summary
At approximately 11:00 A.M., on January 15, 2013, Employee #1 of Salinas Valley State Prison fell off the back of a 'boxtail' lift gate truck. Employee #1 was loading laundry carts in the back of the truck and went to exit the truck via the lift gate but it had been lowered already. Employee #1 did not notice this and fell to the ground. Employee #1 was taken to Salinas Valley Memorial Hospital where he had surgery.
