Investigation Summary
On March 2, 2013, an employee was tending a Midi-Mite Mulch double paddle Coloring machine by Amerimulch. The nozzles for the dye operation became clogged and the employee conferred with the operator of the machine to turn the machine off so that employee could unclog the nozzles. In written statements by the operator and of two witnesses, the operator turned the machine off using the three buttons used to shut down the three parts of the machine. There was no Lock out/Tag out policy in effect. After the operator had shut the machine off, the employee then climbed into the machine to unclog the nozzles. Witness #1 stated that he was approximately 50 feet from the incident said he saw the operator turn the machine "off". When the employee climbed into the machine, the operator left the control panel, walked behind the machine, reversed his course, walked back to the panel and turned the machine back "on." The witness stated he heard the employee begin to scream while the operator walked away from the machine, then climbed began to help but did not turn the machine "off." Witness #2 turned the machine "off" then removed panels to try to get the employee from the machine. Emergency Medical Services were called, and the employee was removed from the machine approximately three hours after the incident. The employee was transported to the hospital, where he died.
