Employee Falls to Floor Level and Is Injured

At approximately 3:55 p.m. on December 28, 2013, Employee #1, a stocking clerk, was working on the sales floor of a discount store. He was stocking shelves from ground level. As he stocked the shelf, he collapsed falling to the floor and sustaining a head injury. Employee #1 was transported to a medical center, where he underwent treatment and was then hospitalized for observation.
