Investigation Summary
On January 6, 2014, Employee #1, a roofer employed by a roofing contractor, was working as part of a crew installing solar panels on the roof of a warehouse. He was positioned in a personnel basket that had been lifted approximately 20 feet from the ground by a forklift. His task was to remove new solar panels from a pallet lifted and positioned next to his basket and to place the panels on the roof. He unhooked his fall protection lanyard from the personnel basket and stepped out of the basket and onto the pallet. As he stepped onto the pallet, which was partially loaded, the pallet shifted. Employee #1 fell to the ground. The pallet also dropped to the ground. Emergency services were called, and Employee #1 was transported to Kaweah Delta Medical Center. He was admitted and treated for fractured legs. This event was reported to Cal/OSHA by the employer on January 6, 2014. The subsequent investigation resulted in the employer being cited for a Serious violation of Title 8 Section 1670(a), which addresses requirements for wearing fall protection.
