Employee Crushes and Amputates Fingers Operating Equipment

At approximately 4:30 a.m. on February 2, 2014, Employee #1 was operating an extruding machine that made sheets of plastic by squeezing the melted plastic between in running rolls. At some point, he put his gloved hand into the unguarded nip point of the in running rolls. His fingers came into contact with the in running side of the rolls of the Welex extruder, line 4. Employee #1 had two fingers amputated from his right hand with two additional fingers crushed.
