Investigation Summary
On January 31, 2014, Employee #1 was using an A-frame 8 foot ladder working on a pylon for an engine installing a rivet into a small hole. He jumped off the ladder and broke his right leg and ankle. Employee #1 stated that he was going down the ladder to reset its position and the ladder shifted and causing it to collapse.
