Employee's Foot Is Crushed When Sheet Metal Falls

At approximately 3:40 p.m. on February 6, 2013, an employee and three coworkers were moving a large table with an approximate 3,900 lbs steel top at the sheet metal shop of the facility. The purpose of the move was to get a wing plank out for shipping. Two 4-ft by 4-ft nominal wood pieces were positioned on one rolling dolly beneath the table to assist in balancing the table during the moving process. As the table was being moved, the steel top inadvertently slipped off the bottom frame and struck the employee. The employee was transported to Holy Cross Hospital, where he was hospitalized and treated for injury to his left foot. He was discharged week and a half later. The causal factor of the accident was the table and steel top was not secured against displacement either by proper piling or other securing means.
