Investigation Summary
On January 25, 2013, Employee #1 of Ulven Forging Inc., was loading 2,500 lb "bail" from the forks of a forklift onto a wooden pallet when the bail began to roll off of the pallet. The bail was approximately 14 to 15 feet long, approximately 8 inches in diameter, and cylindrical in shape. Employee #1 instinctively tried to stop it from rolling off the pallet and onto the ground by sticking his foot out, but the bail was too big and heavy and it rolled onto his right foot and ankle. Employee #1 suffered compound fractures to his right foot and ankle.
