Investigation Summary
On August 23, 2012, Employee #1 was working on top of a parking garage structure installing a solar panel. He was standing on top of a beam when his foot slipped, lost his balance, and fell approximately 11 to 12 feet to the asphalt ground. Emergency personnel responded and transported Employee #1 for a lacerated forehead.
