Investigation Summary
On February 13, 2012, Employee #1, a food preparation worker for the Santa Fe Tortilla Company, was changing out the pistons on the flour rounder and divider machine. The machine unexpectedly started up, resulting in the amputation of four of his fingers. Employee #1 was hospitalized.
