Employee's Finger Amputated While Un-Jamming a Trim Saw

On May 7, 2013, Employee #1 of Cerro Pacific Lumber Inc., was attempting to clear a trim saw which had been partially jammed. Trimmed wood pieces were preventing the unfettered access of incoming boards to the saw. Employee #1 instructed Employee #2, the saw operator, to shut off equipment so that the jam could be cleared. The equipment was shut off but not locked-out or de-energized. Employee #1 caught his right index finger on the saw blade that had not yet spun to a stop. The employer and employee acknowledged that no attempt was made to de-energize and lock out the motor prior to cleaning the saw. Employee #1 sustained an amputation to his right index finger above the distal joint and was transported to a nearby hospital where he underwent medical treatment to address the injury.
