Investigation Summary

Event: 10/09/2012
Employee Falls from Ladder and Receives Fractures

On October 9, 2012, Employee #1, an assistant, was working for a racecar shop and installing promotional banners for an upcoming race at a local racetrack. He stood on a 6 ft self-supporting A frame fiberglass ladder to install the banners. A coworker was also standing on the ladder fastening the grommets of a vinyl banner to a plywood sound wall as Employee #1 held the banner. When the coworker leaned against the plywood wall, the wood moved slightly, pinching Employee #1's fingers. He had his fingers behind the plywood using it as leverage to hold the banner tight against it while the banner was being fastened. When Employee #1's fingers were pinched, he jumped back, lost his balance and fell to the cement walkway below. He suffered a fracture in his left hip and arm. Employee #1 was transported to a medical center, where he hospitalized for five days.
