Investigation Summary
On July 31, 2012, Employee #1, a roofer, accessed a steep roof by climbing through an access hole left in the roof sheathing. He was going up on the roof to install anchor point(s) so tar paper could be installed over the roof sheathing. As Employee #1 was walking along the peak of the roof, he had to go around a wooden chimney box to access the other end of the roof. When Employee #1 went to walk around the chimney box, he either lost his balance or slipped on the roofs sheathing. This accident happened just after daylight hours began and dew was on the sheathing. Employee #1 fell from the edge of the roof to the ground, approximately 33 feet below. He struck his head and face first on a section of concrete gutter. Employee #1 was pronounced dead at approximately 7:05 a.m. at the worksite. The body was transported to the Office of the Chief Medical Examiner for an autopsy.
