Investigation Summary
Employee #1 was performing a non routine task, using lacquer thinner, which contains methanol, toluene, acetone, MEK, petroleum, to remove adhesive from a floor that had just been removed. It appears that the water heater's gas pilot light ignited the vapors, causing an explosion. Employee #1 was hospitalized suffering from burns on on his legs, arms, neck and face.
