Investigation Summary
On February 8, 2012, Employee # 1 and crew were constructing building approximately 30 feet by 48 feet, to be used for wildlife production. The crew was installing ribbed steel panels, approximately 36 inches and 29 gauges, to the roof top area. Employee # 1 used a portable ladder to access an exterior overhang roof top, walk across the overhang, then access the interior structure of the main building. Employee # 1 had climbed down from the roof to a 2 by4 wooden purlin area. He was walking along the 2 by4 wooden purlins when he lost his balance and fell approximately 15 ft. to the ground. Employee # 1 suffered head injury and was hospitalized for approximately two weeks, where he later died. Employee # 1 was not wearing any type of personal fall protection equipment.
